Newborn Questionnaire

Thank you so much for scheduling a photography session with me.  Please fill out the following questionnaire, this will help me be better prepared for our upcoming session.  Your information will never be published or shared. This form is required as part of your booking process. Please let me know if you have questions!

I do 4 Sets for each Newborn session and within those sets I choose the outfits, accents and wraps that match your theming. Is there a specific pose that is important to you? If so, try to discribe it the best you can. *

Bum up, laying on side, head up on hands, wedding rings on feet, baby in basket propped up, etc

SET 1: Themed, this can be anything that uses a prop or themed set, such as pirate, military, holiday, elephants to match nursery, or just a brown stool, etc. * *

SET 2: Themed/Basket, this can be anything that uses a basket or crate prop, flowers, toys, holiday, animals, superheros, princesses, or just plain colors, etc. * *

SET 3: Blanket Set, please choose a color for the second blanket set, this can be any color for the background of the images. Then please list a few styles/colors of outfits, wraps and accents that you would like to use on this blanket. I will choose coordinating outfits and wraps based on your color/style selection. * *

What do you plan to do with your finished images? *

Hang on walls, prints, canvases, albums, etc. This will help me choose the right shooting style before be begin.

If parents or siblings going to be included, would you like a Grey or Cream backdrop? * *

If you choose grey, please come dressed in cool colors, blues, blacks, greys, whites, or any other colors that would look good against a grey backdrop. If you choose Cream, please come in earth tones, whites, creams, tans, browns, and any other coordinating color accent that would look good against a cream backdrop. I will swaddle baby in the best coordinating color to match the parents and siblings.

If you have seen my work on my website, are there any specific props that you "must" have inclued into your session? * *

Keep in mind, props are rotated out frequently and some props get retired, as well as some shots are digital composites, so this question is very important to make sure I have the props for your session.

Your due date is stored on my calendar once you have paid your deposit. Since not all babies arrive on their due date, I will wait until I hear from you on your baby’s arrival. You must notify me the day the baby arrives, then we will schedule the session within 10 days after your baby is born.

Please note that with all this said, newborn babies cannot be pushed or forced into positions where they are not comfortable.  I will do my best to fulfill your wishes with your baby’s safety and comfort in mind first.

If siblings and/or parents are included in the newborn session then we will take those images at the beginning of the session.  Please make arrangements for someone to pick up the siblings since the newborn session can last a few hours and it is important to have a quiet serene environment for the best newborn session.
